๐Ÿ“ฃ AaNeel has met the CMS Kill the Clipboard pledge through its Health Access Cardยฎ workflow.

For years, healthcare has talked about giving patients greater access to their information and eliminating the repetitive paperwork that follows them from one care setting to the next. The CMS Kill the Clipboard initiative is helping move that vision closer to reality.

The Health Access Cardยฎ gives patients greater control over how their health information is shared while helping authorized providers securely access the information they need at the point of care through FHIRยฎ APIs.

AaNeel is now featured in the CMS Health Technology Ecosystem Pledge Showcase, demonstrating how interoperability can move beyond technical infrastructure and become part of the actual care experience.

๐Ÿ’ก Less repetitive paperwork
๐Ÿ” More secure information sharing
๐Ÿฅ Better access to information where care happens

This is an important step toward making connected health information practical for both patients and providers.

๐Ÿงฐ Specialist engagement is not about having more tools. It is about knowing which tool to use for the problem in front of you.

Value based care organizations already have dashboards, referral strategies, care pathways, incentives, and performance measures. The real question is whether those resources are addressing the right barrier.

Inspired by the NAACOS Specialty Engagement Accountable Care Toolkit discussion, Part 1 of our Specialist Engagement Toolbox Series looks at why effective engagement starts with identifying the challenge first, whether that challenge is accountability, data, access, workflow, incentives, or trust.

AI should give time back to care. ๐Ÿฉบ

As artificial intelligence becomes part of everyday clinical work, its greatest value may not be replacing decisions or changing how medicine is practiced. It may be reducing the administrative burden that keeps physicians and care teams away from patients.

In our August edition of 's "Care in Motion" newsletter, we explore how AI can support documentation, prior authorization, care coordination, workflow efficiency, and patient engagement while keeping human oversight, privacy, and accountability at the center.

Because the goal should NOT be more automation for the sake of automation. The goal should be MORE TIME for the people delivering care. ๐Ÿ‘ฅ

๐Ÿฅ Value-based care changed who is accountable for outcomes. But did healthcare's infrastructure change with it?

That question is becoming increasingly important as federal healthcare priorities put greater emphasis on real-time data, interoperability, digital quality measurement, and reducing administrative burden. For ACOs and other risk-bearing organizations, the implications go well beyond technology.

๐Ÿ“Š A care gap identified months later may still be accurate, but there is less opportunity to close it.
๐Ÿฅ A hospitalization discovered after discharge can inform future risk, but it cannot help coordinate the transition home when the patient needs support.
๐Ÿ”— A medication change trapped in one system cannot inform a provider who never sees it.

And then there is "Justin". He does not care about APIs or data architecture. He cares whether his cardiologist knows what his primary care physician changed and whether he has to become the messenger between them.

Our latest blog looks at why real-time healthcare data is becoming part of the infrastructure value-based care has needed all along, and what that shift means for ACOs, providers, health plans, and ultimately, patients. ๐Ÿ“– Read the full blog: https://www.aaneel.com/real-time-healthcare-data-value-based-care/

๐Ÿ”— Better outcomes begin with better information.

The latest discussion around ACO REACH reinforces an important lesson for value based care. High performing organizations need more than financial incentives. They need timely, trusted, and actionable data that helps clinicians coordinate care, identify risk earlier, and make informed decisions across the continuum.

Technology should simplify healthcare, not complicate it.

When interoperability and workflow come together, providers can spend less time searching for information and more time caring for patients. Value based care depends on more than policy. It depends on information that moves with the patient.

As accountable care continues to mature, healthcare organizations are recognizing that success requires more than financial models and quality metrics. It requires trusted data, interoperable systems, and technology that helps clinicians act on information rather than search for it.

The next phase of Medicare reform will reward organizations that can connect people, processes, and technology across the continuum of care. ๐ŸŽฅ In Case You Missed It... Some of the most valuable conversations from TXAACOs 2026 didn't happen on stage. They happened in the discussions between sessions, over dinner, and among healthcare leaders working every day to advance value-based care.

To keep those conversations going, AaNeel hosted an outstanding panel of nationally recognized healthcare executives, physician leaders, and innovators to discuss where healthcare is headed next. Together, they explored:

โœ” What healthcare leaders are most concerned about heading into 2027
โœ” What separates high-performing organizations from everyone else
โœ” Where AI is delivering measurable value and where hype still exists
โœ” Why interoperability remains one of healthcare's greatest opportunities
โœ” The biggest mistakes organizations continue to make
โœ” What CEOs, ACO leaders, and healthcare executives should prioritize now

๐Ÿฅ What if one of the most important predictors of a patient's health isn't found in their medical record?

Healthcare has become incredibly good at diagnosing disease, measuring quality, and identifying clinical risk. But those metrics rarely tell the whole story.

๐Ÿš— Does a patient have reliable transportation?
๐Ÿฅ— Can they afford healthy food?
๐Ÿ  Is housing stable?
โšก Are they choosing between paying for utilities and paying for medications?

These are all Social Determinants of Health (SDOH), and they often have as much influence on outcomes as the clinical diagnosis itself. As value based care continues to evolve, organizations are beginning to recognize that improving outcomes requires understanding not only what is happening, but also why.

In our latest blog, we explore why Social Determinants of Health are becoming one of the most important conversations in healthcare and how better questions can lead to better care. ๐Ÿ“– Read the full blog: https://www.aaneel.com/social-determinants-of-health-sdoh/

Technology is changing healthcare. Interoperability is making it possible. AI is making it actionable.

The CY 2027 Medicare Physician Fee Schedule proposed rule is another reminder that the future of value based care depends on more than reimbursement models.

It depends on whether clinicians have timely, complete, and usable information at the point of care.

The proposed rule continues the shift toward:
โœ” Value based reimbursement
โœ” Population health management
โœ” Better care coordination
โœ” Reduced administrative burden

None of that happens without trusted, interoperable data.

Technology should not create more work for providers. It should remove barriers, surface meaningful insights, and make better decisions easier.

That is where the next phase of healthcare transformation begins.

35 million Medicare Advantage members is not simply an enrollment statistic.

It is a signal. ๐Ÿ“ˆ

A signal that millions of people are receiving care inside a model increasingly shaped by prevention, coordination, engagement and value-based performance.

But growth alone does not create better care.

The real opportunity depends on whether health plans, providers and technology partners can connect data, align workflows and give care teams the information they need across the patient journey.

In our latest blog, we explore what the continued growth of Medicare Advantage says about where healthcare is heading and why the infrastructure behind the model matters more than ever.
