March 2013: a section of a 14-foot trench collapses. The rule — no working in unprotected sections — already exists, on paper. Later that day, the trench collapses again.

Paper only reaches whoever is holding it. A hazard anyone on the crew can log, sign, and date reaches the people who can actually act on it.

See it. Report it. Get it fixed. → crewview.com

1999: a guardrail comes off for a work task. It never goes back on. A worker falls about 10 feet — multiple fractures, hospitalized.

Removing a guardrail is half the job. Replacing it is the other half. Both halves should be assigned, tracked, and closed out with a record.

See it. Assign it. Fix it. Document it. → crewview.com

10 minutes here. 10 minutes there.
It doesn’t seem like much—until you add it up across an entire crew.
