🩻 A radiologist and a general annotator can look at the same CT scan and see completely different things. If it's the general annotator's interpretation that ends up in the training dataset, the model learns accordingly.

Medical image annotation is categorically different from standard computer vision work. The data formats are specialised, DICOM, NIfTI, whole-slide imaging and require tools and clinical metadata handling that general-purpose annotation platforms aren't built for.

The regulatory environment is another important aspect to consider. HIPAA, GDPR Article 9 and FDA documentation requirements all apply to how the data is handled during labeling, not just after.

Tamara Sergiienko spent years working with refugees and migrants, learning to listen carefully, spot what people actually needed, and turn complex situations into clear, useful feedback. πŸ’«

She relocated to Bulgaria, joined HITL as an annotator in 2023, and this year became part of our Beneficiary Advisory Board, where she bridges communication between local annotators and management, making sure the Bulgarian community's voice shapes the decisions that affect them.

When we asked her about the role, Tamara said: "Most of it is just about listening. Understanding the real human experience behind the work, not just the tasks themselves."

Tamara still annotates, which means she knows exactly what she's listening for.

πŸ‡ͺπŸ‡Ί On 2 August 2026, Article 10 of the EU AI Act comes into force for high-risk AI systems. Most regulatory frameworks deal in broad principles and leave teams to interpret what that means for their day-to-day work.

Article 10 is different. It names the exact data preparation processes it governs annotation, labelling, cleaning, updating, enrichment and aggregation. If your AI system falls into the high-risk category, these are no longer just workflow decisions. They are regulated, documentable, and auditable processes with significant legal consequences for non-compliance. βš–οΈ

πŸ”¬ What that means practically: documented task specifications before annotation begins, qualified annotators with traceable profiles, inter-annotator agreement scores as standard pipeline output, and a full audit log for every data point.

βš•οΈ AI adoption in healthcare is moving fast - predictive tools, diagnostic imaging, robotic surgery. What most teams building in this space eventually run into is that the training data doesn't get the same attention as the model itself. In healthcare, that gap has consequences.

Demographic gaps in imaging datasets, inconsistent annotation guidelines, clinical reviewers who disagree on edge cases, all of this ends up in the model before anyone notices.

We've worked on medical annotation projects across surgical robotics, safety, clinical research and medical imaging, with research institutions and medtech companies.
