A near miss is not a lucky escape. It is an unpaid invoice.

No injury, no damage, nothing worth reporting, so it gets filed as a success. But the cost was incurred, it just has not been presented yet. The intersection has the same traffic design tomorrow. The alert ignored once will be ignored again.

One near miss at an intersection is an incident. Twenty at the same intersection over 90 days is a traffic design problem with a location.

A Cape Town studio just won an Emmy for a play performed inside VR.

Uncanny Alley: A New Day took Outstanding Emerging Media Programme at the 78th Emmy Awards on Saturday. Two actors, fifteen characters, an audience of six, performed live in VRChat, inside an original universe built by South African worldbuilder Rick Treweek.

It beat studio-backed entries from Amazon, Blumhouse, Meta and Bethesda.

Read that again. An artist-led immersive world, built in South Africa, just set the global benchmark for the future of storytelling.

This is the thing we keep saying: immersion is not Silicon Valley's game. The craft that just won an Emmy is the same craft we use every day to turn operational knowledge into VR training, simulations and digital twins for mining, industrial and corporate teams. World-class immersive talent is not something we import. It is something we grow here.

Congratulations to Rick Treweek, Virtual Worlds Company and Ferryman Collective. You made the whole industry proud.

Your most valuable asset is about to retire, and it is not on the balance sheet.

Every workforce conversation in mining is about headcount. But what leaves when a veteran operator retires is judgement, the instinct that hears a bearing about to fail before any instrument flags it. That was never in a manual. It was passed down by standing next to someone for years.

The knowledge cliff is not a hiring problem. It is a transfer problem.

The headset is the least interesting thing in the room.

For years training has run on the assumption that information changes behaviour. But your people can already reach any procedure in seconds, and performance is still uneven. Information was never the real problem.

What changes behaviour is consequence. Someone who makes a decision and lives with the outcome builds a practised response. Someone who watches a video builds an opinion.

Every company can tell you exactly what its training cost.
Almost none can tell you what its forgetting cost.
This is not an accounting oversight. It is the whole problem.

A training budget measures delivery. It counts modules built, sessions run, people who clicked "complete". All of it lands neatly in a spreadsheet. Forgetting, meanwhile, files its expenses somewhere else entirely. It shows up as an incident. As rework. As the same induction run twice. As the operator who knew the procedure perfectly and froze anyway.

No line item ever reads "training that did not stick". So it never gets fixed.

We assumed that giving people information was the same as building capability. It is not, and it never was. Ebbinghaus worked this out in 1885 and we have spent 140 years politely ignoring him. Roughly half of what you passively absorb is gone within the hour.

Here is the uncomfortable test. If information were the answer, the best informed companies would be the safest ones.
They are not. Reading about swimming and having swum are not the same skill. The 2025 research is blunt about it: immersive training retains 75 to 90 percent, passive eLearning 10 to 20. The bottleneck was never information. It was consequence.
