Kidney care shouldn't begin and end with a clinic visit.

HHS's new Kidney Care National Plan puts prevention, dialysis, and transplantation in focus.

But how do these priorities translate into better care for patients?

For practice leaders and value-based care teams, the challenge is turning policy into everyday practice.

Read our analysis of what healthcare organizations need to know about HHS's new kidney care priorities.

“Our RPM vendor said we're compliant.”

That's reassuring. But can your team find the patient's readings, explain who reviewed them, show what clinical action followed, and locate the documentation?

Your RPM software may collect readings, flag alerts, and organize patient data.

But can your practice connect that information to the care delivered, the documentation in the EHR, and the claim submitted?

Your vendor may support parts of the workflow.

Your practice still needs to understand how those pieces fit together because it holds liability.
So instead of only asking, “Are we compliant?”

Ask: “Show how our workflow supports the claims we're submitting?”

Here's a question worth asking about your RPM program:
Is another practice billing RPM for the same patient?
Imagine a patient who sees both primary care and cardiology.

Both practices use remote patient monitoring, and each has its own staff, records, and billing process.

Your team may know what your practice is billing. But does it know whether another practice is billing RPM for that patient, too?

Medicare generally allows only one practitioner to bill for RPM for a patient in a 30-day period. CMS guidance

So here are some practical ways to address the blind spot:

- During enrollment and monthly, before delivering care to the patient, ask if they are enrolled in RPM at their other provider's office.

- Build an overlap check into your pre-billing process.

- If a potential conflict comes up, clarify billing responsibility with the relevant practice and document the resolution.

- Submit claims immediately after service and documentation to ensure your practice is first in line for processing and payment.

You may not have visibility into every service another practice provides. But you can build a process that keeps your practice compliant and reimbursed for the RPM services you provide.

The hospital discharged the patient Friday.

The patient is home.
But the transition of care has just begun.

Over the weekend, the patient may be adjusting to new medications, trying to understand new instructions, or waiting for follow-up. Then Monday arrives.

And the clinician at the practice still needs to figure out:
What medications had been changed?
Whether the patient understands the new regimen?
Whether another provider needs to be involved?

Imagine trying to do all of that while the practice is already dealing with a full Monday:

Phones are ringing.
Messages are waiting.
Refill requests are coming in.
Patients are arriving.

That's why transitions of care can become an operational challenge.

TCM gives practices a framework for managing those days after discharge.

But a framework only works when there’s an operational capacity to carry it out.

Without enough capacity behind the workflow, the work starts competing with everything else on the practice's plate.

And that's when things can fall through the cracks.

Read the full article to see how a TCM workflow can give the transition structure, accountability, and a clear path from discharge to follow-up.

Making the post-discharge outreach is important.

But what happens when the patient says:
“I’m taking the new medication, but I stopped my old one because I thought they were the same.”

So what started as:
“Hi, we're calling to check how you're doing after your hospital stay.”
has suddenly become a medication-safety question.

Now the clinical staff needs to figure out things like:
What the patient was taking before hospitalization
What changed during the admission
What the patient is taking now

This is why TCM is more than just reaching out to the patient, It's more like opening a door.
The call gets you inside the patient's real-world situation.
And sometimes what's behind that door is more complicated than the discharge summary suggests.

That's where adding expert clinical support can become valuable, particularly in areas the practice may not have the capacity to cover today.

The patient is home from the hospital.

And for the practice, that's when the real work starts.

A patient is discharged on Friday.
Their medication list has changed.
They aren't completely sure which instructions to follow.
Their follow-up appointment hasn't been scheduled yet.
And Monday morning the practice is already busy.

This is the reality behind Transitional Care Management (TCM). The work is beyond just initiating contact

It's:
→ Reaching out to them
→ Finding out what changed
→ Reconciling their medications
→ Reviewing their discharge plan
→ Arranging a follow-up
→ Coordinating care
→ Documenting what was done

It's a little like changing the engine on a moving car.
The practice still has to keep everything else running while the care team manages a time-sensitive transition.

With CMS requirements continuing to evolve, leaders need to know what a compliant workflow must account for.

Heart health is built between the beats.

On World Heart Day, we’re reminded that cardiovascular health is not defined by a single reading, appointment, or intervention. It is shaped by what happens consistently between those moments.

For patients living with chronic conditions, the difference can come down to the fundamentals: medication adherence, blood pressure control, cholesterol management, early recognition of risk, and continuous care.

The opportunity for healthcare organizations is to move beyond episodic care and build connected models that help identify risk earlier, close gaps in care, and support patients continuously.

The future of cardiovascular care depends on connecting the right clinical insights, interventions, and support across the entire care journey, not just at the point of care.

At PharmD Live, we are committed to advancing that conversation and rethinking what continuous, connected care can look like for people managing complex health needs.

Know your numbers. Know the signs. Don’t miss a beat.

Integrated clinical care is not simply about bringing more disciplines into the room.

It’s about connecting expertise around the patient so that medication decisions, clinical priorities, and care plans work together, not in silos.

For complex and high-risk populations, that coordination can be the difference between fragmented intervention and proactive, informed care.

At PharmD Live, we integrate multidisciplinary clinical expertise and medication management to help healthcare organizations strengthen care coordination, identify risks earlier, and improve outcomes while reducing unnecessary utilization.

Better care happens when the right expertise works together.

Telehealth solved an important problem: Access!

But access alone doesn’t create sustainable outcomes.

For patients managing chronic conditions, the greatest risks often emerge between appointments, when symptoms change, medications need attention, engagement drops, or early warning signs go unnoticed.

That’s why the next phase of virtual care isn't simply more telehealth visits. It’s continuous care: ongoing monitoring, patient engagement, and medication support that extends the care team’s reach beyond the traditional visit.

At PharmD Live, we combine Chronic Care Management, Remote Patient Monitoring, multi-disciplinary care team expertise, and technology to help healthcare organizations stay connected to patients throughout their care journey.
