Health systems collect 95–97% of what commercial payers owe. For patients, that number drops to 30–50%.
The gap isn't because patients won't pay, it's because they're rarely told the price before care happens. As high-deductible health plans push more cost onto patients, and CMS's 2026 price transparency rules raise the bar further, the health systems that make cost and quality visible upfront are the ones that will get paid predictably and win patient volume in the process.
