Having MFA isn't the same as being protected by MFA anymore.

Attackers have adapted. MFA fatigue attacks push approval prompts until a tired employee taps "approve" just to stop the buzzing. Adversary-in-the-middle attacks use a convincing fake login page to relay your password and your code to the real site in real time, then steal the session.

Both defeat SMS codes and simple push approvals. Neither defeats hardware keys or passkeys.
