The 90-day episode continues after the patient leaves. Hospital visibility often doesn’t.

Under CJR‑X, hospitals will be accountable for related Part A and Part B spending through 90 days after discharge - with two-sided risk beginning on day one of the model.

But the utilization that moves episode cost can happen outside the hospital’s immediate view: post-acute care, emergency department visits, readmissions and other services across the care journey.

Closing that 90-day blind spot will require a clearer view of where post-discharge spend is occurring—and an earlier opportunity to respond.
