In May 2021, hackers got into Colonial Pipeline's network using a single compromised VPN password — an old, reused password with no multi-factor authentication behind it, later found circulating on the dark web from an unrelated breach. Colonial paid a $4.4 million ransom in bitcoin (the DOJ later recovered about $2.4 million of it) and still had to shut the pipeline down for five days, triggering fuel shortages and price spikes across the East Coast.

In September 2022, attackers bought a contractor's stolen password off the dark web, then bombarded the contractor's phone with MFA push notifications until, worn down, they approved one. From there, the attacker roamed Uber's internal Slack, cloud infrastructure, and financial tools largely unchecked for hours before being noticed.

𝗧𝗵𝗲 𝗹𝗲𝘀𝘀𝗼𝗻: Not all MFA is equal, and access, once granted, still needs to be watched. Phishing-resistant authentication and real-time monitoring of privileged access are what stop “MFA fatigue” from becoming a full breach.

In March 2019, LockerGoga ransomware — planted weeks earlier through a phishing email disguised as a message from a trusted customer — brought down systems across Norsk Hydro's global aluminum operations. They refused to pay the ransom and recovered using tested backups and manual, paper-based operations. It's held up industry-wide as one of the best-handled ransomware responses ever. It still cost them $71 million.
