The result is long, repetitive, and still missing the comorbidity that matters. One structured assessment covers the ground the packet was reaching for, in less of the patient's time.

When intake captured a structured, DSM-aligned picture, the reentry plan can connect people to the right community care instead of restarting the assessment. Continuity from booking to release is a documentation question first.

Many payers require documented evidence of response partway through a TMS course before authorizing the rest of it, and that evidence has to be measured rather than described. Clinics with repeatable severity measures produce it on request. Clinics reconstructing it from scattered notes risk an interruption in treatment while they assemble what the packet needs.
