In mental health billing, the costliest Revenue Leak never shows up as a denial.
Two losses produce no denial at all:
β Sessions delivered after an authorization runs out. The care happened, the note is solid, and no claim can go out.
β Clinicians seeing patients before credentialing is complete. These sessions are unbillable, not denied, so there is nothing to appeal.
Your denial rate can look healthy while you still lose money.
Both are prevention problems. Both get worse in a Frankenstack, where authorization status never reaches the schedule and enrollment lives in a spreadsheet.
When you evaluate an EHR, ask to see an expiring authorization flagged before the session.
That is Payer Defense, built on One Platform.
