We’ve been sharing a lot about what happens after services end.

Here’s the big picture.

Progress doesn’t just depend on what happens in a program.

It depends on what happens after:
• Are skills still used?
• Do they work in real life?
• Is there support in place?

That’s where outcomes either stick… or fade.

We brought everything together in one place:

Most programs measure success at discharge. But what happens after? Learn how to build outcomes that last beyond the 90-day window.

What happens after someone leaves your program?

For many teams, it’s hard to know.

One simple way to stay connected:
Check in at 30, 60, and 90 days.

It doesn’t have to be complicated.

We shared a simple way to do this:

A lightweight system for post-discharge follow-up that catches regression early and generates evidence that your outcomes actually stick.

Regression rarely happens all at once.

There are usually early signs:
• Routines slipping
• Less engagement
• Small issues increasing

But they’re easy to miss.

By the time it’s obvious, it’s much harder to fix.

Here’s what to watch for:

Turn "we didn't see it coming" into early warning detection. These measurable signs predict regression before full skill loss occurs.

Why do some individuals maintain progress… and others don’t?

It’s usually not motivation.

There are patterns:
• Lack of support after services
• Skills that don’t transfer
• No follow-up plan

Once you start looking for these, you see them everywhere.

We put together a simple framework to help:

Not everyone regresses after services end. Learn the 7 warning signs that predict whether outcomes will stick—and what to do about each one.

Our April Equip update is out.

This month, we’re sharing:
• New product updates designed to reduce admin work
• Our new brand and expanded focus
• A closer look at what it takes to build outcomes that last

We’re also highlighting Oklahoma State’s Opportunity Orange Scholars program and the impact they’re making.

If you’re thinking about long-term independence and how to better track real outcomes, this one is worth a read.

Have you ever seen someone do really well in one setting…
and then struggle in another?

It’s not that the skill is gone.

It’s that the environment changed—and the support didn’t carry over.

This happens more than we talk about.

We broke down why it happens (and what helps):

Skills learned in one setting often fail in another. Understanding why—and what to do about it—is key to outcomes that survive transitions.

Just because someone learned a skill doesn’t mean it will last.

Or that it will work outside your program.

Two things matter:
• Can they still do it later?
• Can they do it in a different setting?

Most programs don’t measure either clearly.

And that’s where outcomes start to break down.

We explain the difference here:

Skills can maintain but not generalize. Or generalize but not maintain. Understanding the difference is key to outcomes that actually last.

Most programs measure success at discharge.

But what happens after?

We’ve all seen it:
Someone makes real progress… and then a few months later, things start to slip.

Not because they failed.
Because the system around them didn’t support what came next.

Over the next few weeks, we’re sharing what actually helps outcomes stick:
✔️ Why progress fades
✔️ What to watch for early
✔️ How to support people after services end

We’re calling this: Outcomes That Stick.
