Improving transitions of care (ToC) is a key in transitioning between fee-for-service healthcare to value-based healthcare systems. ToC management is important for both patients and hospitals, and one of its main functions is the prevention of avoidable hospital readmissions. This can reduce patient inconvenience and more precisely control medical quality and resources.

To prevent readmissions, providers need to be able to closely monitor patients' biodata during crucial periods. Besides allowing providers to ensure patients are stable and taking their medication as prescribed both and home and in transit, potential complications can also be detected and prevented beforehand. By addressing these factors, ToC management can be greatly improved.

FaceHeart Vitals™ provides a convenient and accurate way to measure biodata. Using a patient’s own devices, they can get their vital signs measured in 40-60 seconds. This enables health providers to monitor patients' conditions closely at both transitional locations or at home. Heart rate variability(HRV), is an index that has been gaining a lot of attention in the medical and health sectors. Being a measurement of variation in time between consecutive heartbeats, HRV is a reliable metric to help quantify the nervous system’s response to stress, being picked up as an important biofeedback by market segments including mobile health, mental wellness, behavioral health, and remote patient monitoring.

However, HRV is a complex biometric that can be affected by a variety of factors, including age, s*x, medication use, and physical fitness. This complexity can make it challenging to interpret HRV results and apply them to individual stress management plans, making accurate and accessible monitoring all the more important.

Although there is a nationwide consensus that digital transformation of health & hospital records are pivotal for US healthcare to move towards value-based incentives, there is an understanding that today’s EHR must undergo aggressive reforms in order to overcome its current challenges in delivery. In addition to lowering administrative clutter and reinventing the “clinical note”, active engagement of patients is crucial.

Ideally, by allowing patients to enter their own information into their health record, they are able to subscribe to its upkeep and take agency of their own health. Unfortunately, this proves to be difficult due to high barriers in the training and device standards required for remote patients to produce consistent and reliable medical data.

With just a phone camera, FaceHeart can enable patients to obtain their own vital signs readings easily, intuitively, and reliably from the comfort of their own homes. Not only would this save medical device costs for them, providers would also benefit from the growing pains of learning and teaching a new device. In a step towards seamless health data transfer between patient and clinician, we hope that FaceHeart Vitals™ SDK can become the turnkey solution.
