EHRs steal primary care doctors face time with patients, study finds.

According to new research published in Family Medicine, Primary care providers spend more time on EHR system compared to looking at patients during visits.
Researchers executed an observational study of family physician residents, attendings and ambulatory patients across 982 clinic visits, related to
10 Residency Research Network of Texas(RRNeT) residencies between May and Jun 2015.

The research findings are as follows - 18.6 minutes is spent on clinical documentation in EHR, 16.5 minutes on face-to-face interaction with patients.
The length of time spent on a patient visit was more in case of new patients, number of medications and the like.

The breakdown time is about 3 minutes working in the EHR before entering the exam room, 2 minutes working on the EHR in the room,
7.5 minutes of non-face time and another 6.9 minutes of EHR work outside of normal clinical hours. 64% of visits had EHR work outside
of office hours and 73% of the visits had physicians working with the EHR in the exam room.

The majority of providers worked during lunch, stayed late or brought their work home to complete the EHR work of the day.

The recent study shows that an average of 3.08 hours for office visits and 3.17 on the computer each day. However, the results varied with
a recent U.K. study which has providers spending 9.5 minutes with patients during a visit, compared to 3.3 minutes on the EHR.

“The UK results are steady with observed consultation times throughout six countries in Europe in 2002 of 7.6 to 15.6 minutes total (overall mean, 10.7 minutes),”
the researchers wrote. “Our results convey that U.S. family physicians spend more time working in the EHR than their European counterparts spend in the entire visit.”
These results are not astounding as multiple studies have been done to measure the number of time providers spend on EHR work, compared to time with the patient.

A 2017 study in the Annals of Family Medicine reveals providers spend close to six hours on EHR data entry during an 11.4-hour workday.
This extra time has often been attributed towards physician burnout and a work-life imbalance.

New research determined that providers spend more than half of clinic visits working with their EHRs than they do interacting with patients.

EHR Standards for India

Indian Government has taken a major step in digitizing the health records of Indian people. This major step has advantages like the good interaction between healthcare providers and patients, accurate diagnosis, and treatment. It also helps in maintaining up to date health records, reducing paperwork.

On a major move towards digitization of Healthcare Industry in India, Ministry of Health and Family Welfare (MoH&FW) has put forth guidelines and standards for the implementation of interoperable Electronic Health Records(EHR) in August 2013. EHR Standards underwent revision by taking inputs from the expert committee. EHR Standards 2016 released by the ministry for health care providers throughout India to adopt into their healthcare system. The goal of ministry towards EHR Standards is to have a uniform rule in capturing, storing, retrieving, exchanging and analyzing healthcare data and information.

Standards and Interoperability
The Primary aim of Interoperability Standards is to ensure syntactic and semantic Interoperability of data among medical systems at all time. These standards need to be flexible enough to be adapted in a country like India.

Health Record IT Standards
Identification and Demographic Information of Patients
A Health Record System should have the provision of following types of Patient Identifiers:
1. UIDAI Aadhaar Number - when available
2. Any Central/State Government issued Photo Identity Card Number

Architecture Requirement and Functional Specifications
EHR System should meet the architectural requirement and functional specifications to be clinically valid and reliable, meet legal and ethical requirements and support good medical practices.

ISO 18308:2011 Health Informatics - Requirements for an electronic health record architecture. This specification defines the architecture of a system that processes, manages and communicates EHR information.

ISO/HL7 10781:2009 defines the requirements that shall be met by individual EHR systems. These standards are requirements of good practice for a clinical data warehouse.

Logical Information Reference Model and Structural Composition
A Health Record System should conform to common semantic and syntactic logical information model and structured composition for captured artifacts. In Order for communication to be same across systems data captured has to be standardized.

Medical Terminology and Coding Standards
The Common Medical Terminology and Coding Standards need to be followed in order to express the unambiguous meaning of data captured, stored, transmitted and analyzed.

Data Standards for Image, Multimedia, Waveform, Document
The Health System stores data records and files of various types during clinical encounters. The diagnostic and prescriptive data generated contain an image, multimedia or document which serve as documentary records.

Data Exchange Standards
A health record system must follow HL7 standards for sharing or communicating data with other systems in health ecosystem.

Discharge/Treatment Summary Format
The printed discharge summary format should meet the Medical Council of India prescribed format.

E-Prescription
Pharmacy Council of India (PCI) has prescribed a format for any medical prescription during drug dispensing is prepared. It has to be digitally signed by a registered medical practitioner.

The term “privacy” means only the person(or organization) authorized by the patient can view the recorded data. The term “security” means all recorded data is protected at all times from unauthorized access.

The electronic records generated by health care provider, are held in trust by them on behalf of patients. The patient owns the health information data. The medium of storage is owned by health care provider

Electronic Medical Record Preservation
When a patient dies it is better to follow a 3-year rule of making the record inactive after 3 years of death.

Data Privacy and Security
Security of Electronic Health Information
The security and privacy are mandatory for Electronic Health Record.
Authentication
The Person or Entity seeking access to Electronic Health Record should be authorized to access the information in accordance with the security standards.

Automatic log-off
The access to EHR is forcibly terminated after a period of inactivity. To access the EHR again, the user has to log in again.

Audit log
All the actions related to EHR including viewing should be recorded. All actions based on user defined actions must be recorded. For administrative purpose, a set of recorded audit information should be retrieved and displayed upon request.

Encryption
The EHR must be encrypted and decrypted as necessary according to best available encryption key strength.

The Ministry of Health and Family Welfare is in the process of standardizing codification for health procedures in India and framing MetaData and Data Standards - MDDS for the health sector.

The EHR Standards comprises of Disease Classification, Medicine and Clinical terminology, Laboratory Data Exchange, Digital Imaging and Communication etc for semantic interoperability. Standards are needed for interoperability and scalability across various health care provider in our country.
