I run a telehealthcare company Trinnect. And we have primarilytwo types of customers.

Some are my long time friend. Calls over WhatsApp. Shyam, my friend is hospitalized in (one of the top hospitals in Mumbai). She has serious stomach issues. Cannot eat for nearly a week. What happened to her?

Me: well, I am not a doctor. I can't tell. Do you want a consultation? As you know quite a few top doctors are associated with us.

My friend: oh, you might have learnt something from your doctors? Can't you tell what happened? The patient party is very anxious to know!

Me: well, upload the documents on our portal. In a few hours, one expert will consult you.

My friend: how much is the fees?

Me: 500 to 700 rupees, depends on the time of consultation.

My friend: ok, let me ask them (patient party).

The story end there.

Another case:

A long time acquaintance:

Hello sir !

Me: hello good morning.

He: here are my documents. Can you please check if the treatment is right?

Me: I am not a doctor. Consultations are done by our doctors. If you want to get a consultation, please let me know.

He: sir, I will call tomorrow.

And that tomorrow never came.

A lady calls me: hello!

Me, hello, how are you?

She: I have a problem, I suspect UTI. (urinary tract infection)

Me: can you upload your complaints?

No word.

Next day she calls: hello, I am in Chennai, visiting **** hospital. Will connect when I am back

During my daily strolls on the station platforms, someone mildly known calls me:

Hello Shyam da!

Me: hello!

He: you know I have a heart problem. Can I consult a cardiologist?

Me: for sure you can. We have excellent specialists of internal medicine as well as sub specialists in interventional cardiology.

He: you know, I often go blank fors while when I get up from sitting. Yesterday it happened while my wife brought me tea.

Me: I am not a doctor, but we have a cardiologist from mumbai.

He: my son bought an apartment in Mumbai. It's big.

Ok. You can get back asap.

Five days, he is seen chewing khaini. But no contact for a consultation ...

Cancer screening

On 26th September 2024, we conducted a Mammography and human papiloma virus (HPV) screening camp in Naihati in collaboration with Chittaranjan National Cancer institute (CNCI). CNCI brought the latest mammogram machine in a big truck and also HPV screening kits.

We could not make a very thorough and widespread advertisement, and 26th September was also very rainy. We could only perform 19 instances of tests. Mostly advised by one single gynaecologist from Naihati. Yet, at least 5 remarkable cases have been found. And these would be followed by CNCI and for free.

In the ambience of memes, rabindrasangeet and Satyajit Roy and biryani and hyper religious atmosphere that prevails in Naihati, it is not very usual that people would bring CNCI and their latest technology for cancer screening. Consequently, I have faced some interesting situations.

It appears, people often are oblivious of CNCI. Particularly as a central government facility specific for cancer treatment. Quite a few were curious if CNCI is sort of a nursing home in the locality.

One local doctor (let this person remain anonymous) informed that CNCI response is very poor. Can I request Dipanwita Banerjee to look at it?

When I handed results to one lady who would require further investigations and monitoring in the coming days, her seniors assured her to take to a private medical facility. I informed them that CNCI perhaps have the best gyno- oncologist in this region, they looked at me with absolute disbelief.

Before the camp, I asked a friend's wife to attend the camp. She nonchalantly said, she would send her domestic helps. As if ladies of her social status do not get cancer or the facilities brought by CNCI are substandard and socially not acceptable.

Right now, two families close to me have members diagnosed with cancer at very late stages. But both cervical and breast cancers can indeed be detected early with a very high chance of cure.

Would you guys not wake up?

আপনারা এসে পরীক্ষা করে নিলে যদি কোনো সম্ভাবনা না থাকে তবে যে মানসিক স্বস্তি পাবেন তা অকল্পনীয় .আর যদি প্রাথমিক পর্যায়ে ধরা পড়ে তবে চিকিৎসা অনেক কম জটিল এবং সেরে ওঠার সম্ভাবনা অনেক বেশি হবে.

We had the first baby steps to tele cardiology in 2018, courtesy Prof. Anil Grover and Dr. Mandeep Singh Kohli. But we were much ahead of time.

The telemedicine guidelines came in 2020 and the same year also the tele health guidelines of American College of Cardiology. However, public perception about telemedicine as of today is primarily telecalling. Digital health is still a couple of years ahead in the future in India.

However, it is worthwhile to start working on it. To start with prevention aspects. The biggest barrier is of course, psychological. If cardiovascular diseases are prevented, then we have a smoother and unremarkable life. Not much to talk about hospital incidences. Not much about thrilling doctors visits. Not much about bedsores.

But a restricted life. Who wants that?
