When MTTR improves, it’s easy to see it as progress. But there’s another question worth asking:

Are the same incidents still coming back?

A team can become faster at resolving recurring incidents without actually reducing recurrence.

Try looking at your top 10 incident types and compare:

Close Time ↔ Recurrence Rate

The results may reveal a very different picture of operational performance.

Because sometimes the goal shouldn’t be to close the next ticket faster.

It should be to prevent the next ticket from happening.

A resolved ticket doesn't always mean the process improved.

If the same issue returns and the team performs the same reset, reboot, reinstall or workaround again, the incident may be closed — but the work is still repeating.

The bigger opportunity comes after resolution:

Can the fix become reusable knowledge?
Can the response be standardized?
Can part of it be automated?
Can the issue be prevented from returning?

Not every recurring incident needs automation.

But recurring work should make us question whether the process is actually learning.

Your system says everything is under control. The dashboards are green, workflows are complete, records are populated, and reports are available.

But what happens when teams still check spreadsheets, search email threads, manually confirm ownership, or ask someone else before trusting what the platform says?

That is where the hidden trust gap begins.

A platform can be technically available while the real verification work continues outside it. Leadership may see completed workflows and healthy dashboards, while employees experience outdated information, private trackers, unclear ownership, and manual workarounds.

The real question is not simply whether the system is working.

A system of record is not automatically a source of truth.

The platform may contain the records.

The workflow may show “complete.”

The dashboard may look healthy.

But when an important decision has to be made, teams may still check a spreadsheet, an email thread, another team, or someone’s personal knowledge.

That is where the trust gap becomes visible.

The real question is not only whether the system contains the information.

It is whether people trust that information enough to act on it without checking somewhere else first.

What usually causes that trust to break first?

Data quality?
Unclear ownership?
Workflow adoption?
Governance?

Sometimes everything stays live.

The dashboard stays green.
The workflow keeps moving.
The records remain populated.

Then an important decision has to be made.

Someone opens a spreadsheet.
Someone checks an old email.
Someone asks another team whether the information in the platform is still correct.

The system is working.

But the operation is relying on something else.

That creates an uncomfortable contradiction:

Leadership sees control.
Employees experience uncertainty.

So where does the truth actually live?

Inside the platform — or inside the workarounds built around it?
