Late last year a runaway 268-car train fully-laden with iron ore was deliberately derailed as it hurtled uncontrollably towards Port Hedland after reaching speeds of up to 162 km/h and resulting in significant asset damage and loss of stock. This accident, like most, is anticipated to have had a number of triggering events and breakdowns across multiple systems. However, what sparked our interest in this story was a mention in the ATSB’s preliminary report that, amongst other things, a maintenance crew had commenced securing the wrong train which had come to a halt on an adjacent track – an observation that makes for a thought-provoking discussion on the importance of accurate in-the-field.
