Observations at 6am. Again at 2pm. Again at 10pm.

It gets called monitoring, but it is closer to sampling. Three or four snapshots across twenty-four hours, and a lot of hours in between that nobody has any information about.

Most of the time that is fine. The problem is that deterioration does not wait for the schedule, and by the time the next round comes around a resident can be well down a path nobody saw them start.

The chart will look complete. Every entry signed, every time recorded, nothing skipped.

That is not a staffing failure. It is a gap in how the model works.
