More than 10,000 critical medication scans.

That's what EMS clinicians have run through eMACC™ since it went live in December.

Here's what the field data looks like:

10,000+ critical scans — numerous verifications per day

Approximately 21 agencies now adopt eMACC every week

Monthly scan volume roughly doubled between February and May 2026

A permanent audit record for every single scan. A verbal cross-check leaves no reliable audit trail at all.

Fentanyl is among the most frequently scanned medications — the drug Dr. Paul Misasi's original MACC study singled out, with a 71.1% reduction in observed error

The MACC was never the problem. Dr. Misasi proved the cross-check works — 49% fewer observed medication errors overall.

The limitation was operational. A verbal cross-check needs a second qualified provider. It competes with scene noise and time pressure. And when it's done, it leaves nothing behind you can audit.

eMACC implements that same sequence in software and removes the ceiling. One provider scans the vial. The system verifies drug identity and concentration, ties it to the weight-based dose, and records the whole thing automatically.

For two-provider crews, it structures the cross-check. For solo medics, it delivers a verification the verbal MACC cannot provide at all.

83% of those scans started from inside the protocol workflow — not a separate checklist, not a bolt-on. Safeguards that live outside the software get skipped under stress.

Our co-founders Tristen Hazlett and Colton Hazlett sat down with The JEMS: Journal of Emergency Medical Services Report to talk about where OneDose started and where it's headed.

It began as a hardware project in their parents' basement. That became OneWeight, a smart gurney scale, and has since grown into a full medication safety platform for EMS under the OneDose brand.

The conversation gets at the why. Medics have been asked to do high-stress mental math in the field for years, with less technology than the hospitals they hand off to. We're out to change that.

The medication-error rate most EMS agencies cite is roughly 17 per 10,000 administrations.

That number comes from self-reporting.

When researchers reviewed structured records directly instead, they found closer to 9.9 safety incidents per 100 encounters — against 0.3 per 100 actually captured in incident reports.

That's about a 33x gap between what gets reported and what's likely happening in the field.
