Community Habilitation Worker provides support to individuals with intellectual/developmental disabilities.
Responsibilities:
Responsible for facilitating the individual’s independence and community inclusion
Responsible for working with individuals to achieve valued outcomes set forth in their community habilitation plan as well as their individualized service plans
Responsible for assisting and supervising the individual in the environment of their choice
Responsible for overall protective oversight of the individual based upon their needs
Provide assistance with personal care skills as necessary; Advocate with and for all supported program participants.
Assist the individuals to achieve and exceed their personal goals.
Demonstrate the ability to effectively teach skills to individuals.
Communicate effectively with individuals, families, and community relationships.

WE WANT YOU! we are looking for energetic individuals, with knowledge in sports, creativity and patient to work with autistic individuals in order to promote independence, self-esteem and community integration in their lives. Direct service professionals work in a support team environment with another direct service team. This is an excellent position to grow - we offer free portable training and certifications!
REQUIREMENTS:
• You must possess a high school diploma or equivalent (such as a GED) or higher.
• You must possess a valid NYS driver's license.
• Must be able to work evenings, weekends and holidays.
• Excellent writing and communication skills.
• Great opportunity for college students majoring in Human Services and Psychology.
• Experience preferred but will train.
