MACg vs OpenEvidence: A Case-Based Comparison for Clinical Decision Support

We have been thinking a lot about what makes AI genuinely useful in clinical care. It's not enough for a system to produce a medically plausible answer. For AI to be valuable at the point of care, it needs to help clinicians move from information to action, with recommendations that are evidence-based, specific, and practical.

We hypothesized that MACg's robust PubMed search capabilities, use of advanced AI models, citation-backed traceability, and ability to acknowledge when sufficient information is not available would make it a valuable tool in clinical workflows.

That's why we conducted a case-based evaluation comparing MACg with OpenEvidence using the same primary care scenarios from a published assessment.

What we found was encouraging. In most cases, both platforms arrived at the same high-level clinical conclusion. But MACg consistently went further by providing more actionable guidance: clearer prioritization of the next step, stronger integration of guidelines and published literature, more attention to monitoring and implementation, and better adaptation to the details of the individual case.

In one case, MACg also flagged an inconsistency in the source narrative, reinforcing an important point: the future of clinical AI is not just retrieval. It's reasoning, context, and judgment support.

Our view is simple: the best clinical AI will not replace physician decision-making. It will strengthen it by making evidence easier to apply in the real world.
