Our HP449 supports QOF 2026/27 achievement for the new obesity indicators OB004 and OB005. When a BMI is entered, the protocol assesses whether the BMI meets the threshold for action under OB004 or OB005.

If the BMI meets the threshold for OB004, the protocol alerts the user that the patient may need referral to a weight management service, or that a reason for not referring should be recorded.

The options offered include:
๐Ÿ”ต referral to weight management service
๐Ÿ”ต weight management service unavailable
๐Ÿ”ต unsuitable for weight management programme
๐Ÿ”ต referral declined

For OB005, the protocol checks whether the patient has the required comorbidities that support eligibility for weight management pharmacotherapy, including tirzepatide. Where the patient has 4 out of 5 relevant conditions, the user is alerted that the patient meets criteria and is prompted to complete the relevant actions. Where the patient has 3 out of 5 conditions, the protocol reviews the record to identify whether the patient may be close to eligibility but has missing or incomplete coding.

Examples include:
๐Ÿ”ต raised blood pressure but no hypertension diagnosis
๐Ÿ”ต raised HbA1c but no diabetes diagnosis
๐Ÿ”ต QRISK above 10% but no statin/lipid-lowering therapy
๐Ÿ”ต previously abnormal lipids but no current-year bloods

This helps the clinician decide whether further review, coding or investigation is appropriate.

Our HP345 supports QOF optimisation for 2026/27 by helping users identify patients where frailty coding may affect achievement of blood pressure and diabetes indicators. This is live for our EMIS customers and is a base Handy Protocol.

Frailty affects blood pressure targets across CD001, CD002, HYP010, HYP011 and DM036. It also affects HbA1c targets for DM020 and DM021, where patients coded as frail may have higher HbA1c thresholds.

The protocol checks whether the patient has outstanding QOF indicators where frailty may affect achievement.

If relevant indicators are outstanding, the alert:

๐Ÿ”ต tells the user which QOF areas may be affected
๐Ÿ”ต highlights blood pressure indicators where moderate or severe frailty may remove target requirements
๐Ÿ”ต highlights diabetes indicators where frailty may affect HbA1c targets
๐Ÿ”ต identifies where Rockwood frailty coding has been recorded but may not count for QOF
๐Ÿ”ต shows the latest QOF frailty code
๐Ÿ”ต shows the latest Rockwood coded frailty entry
๐Ÿ”ต shows the latest Rockwood frailty score/value
๐Ÿ”ต links to a template so users can review and update frailty coding where appropriate

๐Ÿ“ข Weโ€™re releasing a new set of QOF 2026/27 resource updates across our OneTemplates, OneMonitoring and Handy Protocols for our EMIS customers.

These updates are designed to help practices identify outstanding QOF activity earlier, improve coding accuracy at the point of care, and support clinical teams with new and updated indicator requirements. It also includes support for updated and new indicators including AST014, AST015, COPD010, NDH003, CD001, CD002, OB004, OB005, HYP010, HYP011, DM037 and HF009.

OneMonitoring has also been updated to align with the latest QOF logic, including revised register handling, frailty updates, DM037 8 Key Care Processes, new obesity requirements and refreshed โ€œdue nowโ€, โ€œout of rangeโ€, โ€œjust needs doingโ€ and โ€œdue by year endโ€ prompts.

Alongside this, weโ€™re releasing five Handy Protocols to support point-of-care review and coding accuracy:
