I've seen it more times than I can count, a campaign goes live, the sales team isn't briefed, the customer service team doesn't know what's being promised, and leadership is measuring completely different outcomes to the marketing team.

The campaign visuals say it all, Heinz products casting shadows shaped like Cinderella's Castle. Simba lifted by a ketchup bottle. Mickey made entirely out of ketchup.

I've worked on projects where I was absolutely certain we had everything covered. Then someone on the team flagged something I'd completely overlooked. Something that would have cost us real time and money if it had gone live.
